Coaching Agreement

As a condition an individual receiving coaching services (Client) from an individual assigned to provide coaching services (Coach) by Pay Forward Coaching (Pay Forward), Client agrees as follows:

Description of Coaching

Pay Forward offers coaching services at no cost to Client. Coaching is a partnership (defined as an alliance, not a legal business partnership) between Coach and Client in a thought-provoking and creative process that inspires Client to maximize personal and professional potential. It is designed to facilitate the creation or development of personal, professional or business goals and for Client to develop and carry out a strategy or plan for achieving those goals. 

Coach Client Relationship

Client is solely responsible for addressing their own physical, mental and emotional well-being, and creating and implementing their own decisions, choices, actions and results. As such, Client acknowledges and agrees that the coaching services are  provided AS IS with no representation or warranty by Coach or Pay Forward. Further, Client agrees that neither Pay Forward nor Coach has any responsibility or liability to Client or any other party for any action or inaction or for any outcome of the coaching services.

Client acknowledges and agrees that coaching is not therapy and does not substitute for therapy, and does not prevent, cure, or treat any mental disorder or medical disease.

Client acknowledges and agrees that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association. Client acknowledges and agrees that coaching should not be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice from legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. 

Client understands that in order to enhance the coaching relationship, Client agrees to communicate honestly, be open to feedback and assistance and to create the time and energy to participate fully in the coaching services. 

Services 

The parties agree to engage in one or more coaching sessions (with any additional sessions to be determined solely by Pay Forward and/or Coach) conducted by video call or other format  as determined by Pay Forward or Coach. Pay Forward Coaching also provides a free Slack community with a team of coaches available to provide support and tactical advice on career situations.

Confidentiality 

This coaching relationship, as well as all information (documented or verbal) that Client shares with Pay Forward or Coach as part of this relationship, is bound by the principles of confidentiality set forth in the ICF Code of Ethics. However, Client acknowledges and agrees that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege. Coach agrees not to disclose any information pertaining to Client except as necessary within Pay Forward to provide the coaching services without the Client’s written consent. 

Cancellation Policy Client agrees that it is the Client's responsibility to notify the Coach at least 24 hours in advance of the scheduled meeting to cancel or reschedule. Coach will attempt in good faith to reschedule the missed meeting. If the meeting is canceled without 24 hours notice or two consecutive meetings are canceled, Coach may elect to not reschedule the meeting.
